The fund "I hope and I trust" initiates the program on introduction and use of a modern technique of vascular access with the help of implantation ports in the course of treatment of infantile oncology diseases. Today in Ukraine the chemotherapy is carried out by means of use of peripheral catheters (the so-called «teflons») and central venous catheters (subclavicular catheters). The use of «teflons» implies repeated painful piercing of veins of small patients and also input in small vessels irritating chemical medication; as a result, veins hide, burst and hematomas are formed in place of punctures. The use of subclavicular catheters excludes the necessity of repeated pricks, but it causes inconveniences for patients because of presence of a sizable external part. Besides, both methods have the so-called "open" access to a patient’s vein, and it does possible penetrations of infections. It has to be admitted that infection and its consequences are dangerous for children with no immunity because of treatment.
Modern experience testifies that stable and safe access for treatment is required to carry out chemotherapy. In the developed countries such access is carried out with the help of implantation systems of constant access (ports) which are established under a skin in a place convenient for the further use. Implantation systems of constant access (ports) have been offered for use in medical practice by Dr. Muller (USA) in 1984. The statistics of their use testifies the advantage and convenience of a similar technique: Western Europe – more than 250 000 systems in a year, North America – more than 400 000 systems in a year. The port consists of the titanic tank covered with bioinert polymer, above which there is a membrane from multilayered silicone through which a special needle preparations are injected by piercing a skin and a membrane. Catheter is attached to the port. It constantly stays in the central vessel or in an artery. The system is completely separated from an environment. The use of ports during treatment has the following advantages in comparison with traditional methods of access: * The port "is completely hidden" under a skin owing to what the risk of infection decreases * Injections are carried out quickly and without serious consequences, excepting regular stresses before a prick * It does not create discomfort for a patient * It allows to inject preparations of irritating action repeatedly * It allows to carry out regular blood samplings for the analysis * The port can be used for 5 years without necessity of reinstallation * Simplicity and convenience of use gives the chance to carry out injections even by parents of a child independently after necessary instructions. The technique of installation of port-catheters is well-tested and is considered to be safe, providing the high level of comfort to a patient. Besides, the given technique demands smaller care of the port. Several Ukrainian doctors from Kiev, Dnepropetrovsk and Lvov were invited to Paris for training by the French firm-manufacturer "PEROUSE". Elena Viktorovna Shajda, the scientific employee of department of children's oncology of National Institute of Cancer is among the doctors having the certificate of the European sample: «Depending on reports and the treatment scheme number of daily injections for one patient can reach 50. In the absence of a port, a subclavicular catheter is inserted to the child. But it is more dangerous and traumatic. Even catheters of high quality can inflame after a month of use. A port is inserted once and can be there for a year or two, or three by practicing elementary hygiene. The probability of infection of the port is 14 times less in comparison with the best subclavicular catheter. I extracted the port after a year of standing - it was absolutely sterile, as if just now from a packing. A port is completely hidden under a skin. If the person has the implanted port - he can bathe in a pool. Children like to move, like to play; the child can be washed, providing him a full hygienic regimen. It is impossible to do this with a usual subclavicular catheter. Servicing a port is easier than traditional systems of access. A port can to be washed once in 4 weeks. A subclavicular catheter is necessary to be washed twice a day to prevent its thrombosing. (A special medication is inserted into blood at each washing!). A port is very elastic, very thermolabile. It does not cause bedsores on a vein and thrombosis which can be caused by a usual subclavicular catheter. The technique of venous access is an ordinary surgical and anesthetic manipulation. The technology of installing a port is not difficult. The US-device on which it is possible to observe vessels is necessary for installation. It can be the simplest and cheapest device which is present in a medical institution. Also the X-ray-device is necessary for the control of correctness of installation of a port. The components are attached to a port – either needles, or adapters to needles, the so-called lines of connection as usual needles are not used. The port is installed once for a long time. A range is from infants to an adult patient. I have already had an occasion to train several doctors from Vinnitsa, Dnepropetrovsk and Ivano-Frankovsk in a technique of installation”. At National Institute of Cancer operations on installation of ports are held for a few years. Oksana Anatolevna Dekanchuk, the mother of the boy Sasha from Belaya Tserkov who is passing treatment, speaks about reaction of the child to traditional methods of venous access and implantation port: «At first we installed «teflons». They are reinstalled endlessly, veins burst and hide. Child’s arms were in bruises. Veins already could not be found. And it is necessary for the child to change «teflon» 4 times a day. We inserted a subclavicular catheter. We did the first session of chemotherapy well and went home. And during 2-3 days we had troubles. The child had a temperature 40 during 2 days. On the first day we called to the ambulance, they gave antifebrile. On the second day we called to the ambulance, and Sasha said that he was feeling cold. And in a flash the child became grey, his lips and nails were dark blue, and his body has started to shake. I started to call to the institute of cancer to Sergey Vladimirovich (attending physician), I ask what it can be. He answered that the catheter had flamed in a vein. He said to us to come immediately, because it was necessary to remove it, and that it could result in a failure. I washed it, nothing emitted anywhere, and nevertheless. We were in Kiev in the morning, they removed the catheter. The temperature went down till afternoon, nervous trembling stopped. A subclavicular catheter is installed without looking, and while installing a port they look into the vein and try to get into a vein as better as possible. We have a port since May. Great, this port it is very convenient. Well, there is small scar but in two weeks it will be ok. But now Sasha can already take a bath. It is possible to change clothes of the child. And with «teflons» or subclavicular catheter it is necessary to disconnect cords constantly in order to change clothes. There is no problem to make an injection or to take blood.
The average cost of implantation systems of constant access (port) (manufacturer Perouse medical) – 280 EUR for one port depending on modification.
Annually with the diagnosis Oncology more than 1000 children are hospitalized.
Not less than 50 % the hospitalized need the installation of a port. The fund "I hope and I trust" begins the program on acquisition and installation of ports of access to patients of children's departments of oncohematology.
